Registration of residence – EU/EFTA nationals
Residence up to 3 months
Citizen of European Union can enter Poland and stay within its territory up to 3 months on the basis of valid travel document (passport) or other valid document confirming his/her identity and citizenship.
Over 3 months
An application form (DOCX) for the registration of residence should be submitted in person to a voivode having jurisdiction over place of residence (Wojewoda Kujawsko-Pomorski), no later than on the next day following the end of the period of 3 months after the entry into the territory of Poland.
PESEL number
PESEL number is required for all who want to stay in Poland for more than 30 days. It will be required e.g. by the tax office, ZUS (Social Security Agency) or even at GP
Registration of stay
If your stay is longer than 3 months, you need to register your stay in Poland.
Student/ PhD student
1) Fill in the form from your dormitory (you can find it in the administration office of your dormitory). If you are renting a flat you just need the rental agreement and your passport and/or resident card.
2) Register online for a visit. Go to the city hall (Shopping Center Toruń Plaza - Address: Broniewskiego 90, 87-100 Toruń) with the documents from 1.a., your passport and/or Residence Permit card. At the City hall you have to complete application:

Employee

Health insurance
Health insurance is provided by NFZ – National Health Fund
Students
Health insurance is mandatory for all students for the whole duration of their stay in Poland.
EU citizens are eligible for free health services in Poland providing that they present a valid European Insurance Card.
Non-EU citizens must sign the insurance agreement with the National Health Fund and pay monthly contribution to the NFZ.
To get NFZ insurance you will need to:
Step 1. Get a proof of residence (in Polish: ZAMELDOWANIE)
Step 2. Necessary documents:
- your passport with a current visa or residence permit
- the documents from the step 1.
- the confirmation of the student status (issued by the student office of your faculty)
Step 3. Go to the National Health Fund (NFZ – Narodowy Fundusz Zdrowia) at (Toruń, Szosa Chełmińska 30 and it is opened from 8 A.M. to 4 P.M.) in the main hall where you can sign the agreement with the NFZ
Step 4. Fill in the ZUS ZZA form, and submit it to an appropriate Polish Social Insurance Institution (ZUS) branch or inspectorate (the form is available on the website of the Polish Social Insurance Institution (ZUS)
The form can be printed and delivered in person, or sent by post to the branch or the Polish Social Insurance Institution (ZUS) inspectorate in the competent district. The cost of the insurance of international students who are not citizens of EU is around 11 EUR ( 55,80 PLN).
Employees
Health insurance is obligatory in Poland. The employer registers the employee with the social security system after they have completed the relevant form (check the employment procedure). EU/EFTA citizens are additionally advised to obtain the valid European Health Insurance Card before arrival.
EU citizens without health insurance in their countries of residence and non-EU citizens can sign a health insurance agreement with the National Health Fund (NFZ – Narodowy Fundusz Zdrowia) or with one of the private clinics.
Legalisation of stay
Legalisation of stay – non-EU/EFTA nationals
Entering the territory of Poland:
Citizens of non-EU countries may enter Poland and stay within its territory on a basis of Schengen or national visa. Visa is issued by a consul.
Temporary residence permit:
The application for a temporary residence permit is submitted in person, no later than on the last day of legal residence in Poland to a voivode having jurisdiction over place of residence (Kujawsko_Pomorski).
Procedure for submitting application:
The application shall be submitted in person, not later than on the last day of legal residence on the territory of the Republic of Poland.
When submitting the application, the foreigner is obliged to submit fingerprints.

and the necessary documents: 1. The application form for a temporary residence permit completed in accordance with the instructions (all boxes), signed legibly with the name and surname in Latin characters; 2. 4 current “biometric” photographs – intact, colour, 35 x 45 mm in size, taken over the last 6 months on a uniform bright background, with good sharpness and showing clearly the eyes and face from the top of the head to the upper part of the shoulders, so that the face occupies 70-80 % of the photographs. The photographs are intended to depict a person without a headgear and dark-lensed glasses, looking directly ahead with eyes open, hair removed from face, a natural expression on a face and a closed mouth; 3. Photocopy of a valid passport, all filled pages (original available for inspection at the office)
Additional documents :
- certificate of study (together with student’s periodic achievement form);
- health insurance;
- confirming the possession of sufficient financial resources to cover the costs of education, living and return – after deduction of the costs of living (i.e. PLN 776 net – a person running household alone, PLN 600 net – a person in the family, per family member – costs of living, PLN 200 or PLN 2500 – for return);
- confirming the amount of the costs of living (lease agreement, statement of the owner of the premises, certificate from the student’s home, etc.)
- proof of payment of tuition fees, if they are chargeable.
Once you have received your temporary residence permit, attach:
- fee for issuing a residence card – PLN 25 Kujawsko Pomorski Urząd Wojewódzki w Bydgoszczy, Biuro Finansowo-Inwestycyjne, bank account no: 56 1010 1078 0000 4222 3100 0000
- certificate of registered residence.
