Candidate’s declarations and obligations

1. Niniejszym oświadczam, że spełniam wymogi określone w Regulaminie konkursu na staże studenckie w ramach programu CO-OP na Uniwersytecie Mikołaja Kopernika w Toruniu.

I hereby declare that I meet the requirements specified in the Rules and Regulations of the competition for student internships within the CO-OP program at Nicolaus Copernicus University in Toruń.

2. Jestem świadoma/-y, że program CO-OP jest realizowany w ramach programu „Inicjatywa Doskonałości – Uczelnia Badawcza” zwanego dalej „IDUB”.

I am aware that the CO-OP program is implemented within the framework of the "Excellence Initiative - Research University" program hereinafter referred to as "IDUB".

3. Oświadczam, że zapoznałam/-em się z Regulaminem konkursu na staże studenckie w ramach programu CO-OP na Uniwersytecie Mikołaja Kopernika w Toruniu i zobowiązuję się do przestrzegania jego zasad.

I declare that I have read and understand the Rules and Regulations of the competition for student internships under the CO-OP program at Nicolaus Copernicus University in Toruń and I undertake to abide by its rules.

4. Jestem świadoma/-y, że obecność na stażu jest obowiązkowa.

I am aware that attendance at the internship is mandatory.

5. Ponadto, przyjmuję do wiadomości, że stażysta może zostać wydalony przez pracodawcę z podanych niżej przyczyn:
a) stawienie się na stażu w stanie nietrzeźwym lub po przyjęciu innych środków odurzających,
b) utrudnianie pracy pracownikom pracodawcy poprzez nieprawidłowe zachowanie podczas stażu,
c) nagminne spóźnianie się na staż,
d) inne objęte przepisami prawa.

I furthermore acknowledge that the intern may be expelled by the employer for the following reasons:
(a) appearing at the internship in an intoxicated state or after taking other drugs,
(b) obstructing the work of the employer's employees through improper behavior during the internship,
(c) repeated lateness to the internship,
(d) others covered by the law.

6. Zobowiązuję się do dostarczenia wszelkiej dokumentacji wymaganej w konkursie na staże studenckie w ramach programu CO-OP na Uniwersytecie Mikołaja Kopernika w Toruniu.

I undertake to provide all documentation required in the competition for student internships under the CO-OP program at Nicolaus Copernicus University in Torun.

7. Oświadczam, że podane w niniejszym wniosku informacje są kompletne i zgodne ze stanem faktycznym.
